Next, we ask you to complete our Client Intake Questionnaire. We want to make sure your Dream Boudoir Experience is everything you dream it can be and more. This questionnaire helps us know what you want to get out of your Dream Boudoir Experience and helps us plan for it to ensure that it will be fun and memorable.

So, please fill it out even if you’ve had a Dream Boudoir Experience with us before. Your answers may change this time. Plus, we might have added some new questions since your last Dream Boudoir Experience.

Be completely honest as you answer the questions. You’ll get more from your Dream Boudoir Experience if you are.

Phantasy Photos Branded Shirt

Your Dream Boudoir Experience includes a Phantasy Photos branded shirt. Please answer the following three questions so we can have your shirt for you when you arrive for your photo session.

HAIR & MAKEUP

Click the look you want for your experience. Makeup service is included with your boudoir experience. While you may choose to do your own, we recommend that you let our professional makeup artist do your makeup. There are no discounts if you choose to do your own makeup.
Click the makeup look you want for your second look. Two (2) makeup looks are included with our Supermodel Experience. We recommend a natural, neutral, natural/neutral glam, or soft glam look for your first look. We recommend a glam, super glam, glitter glam, or shimmer glam look for your second look. That said, the choice is yours.
Click the hairstyle you want for your experience. Hairstyling is included with your boudoir experience. While you may choose to do your own, we recommend that you let our professional hairstylist do your hair. There are no discounts if you choose to do your own hair.
Click the hairstyle you want for your second look. Two (2) hairstyles are included with our Supermodel Experience. We recommend a blowout, downstyle, downstyle braided, or half up, half down hairstyle for your first hairstyle. We recommend an upstyle hairstyle for your second hairstyle. That said, the choice is yours.
Do you have allergies to any skin-care, makeup, hairstyling, or other beauty products, including latex allergies?
This is an important question. We need you to answer honestly. We would hate for you to have an allergic reaction to the makeup or hair products our professional makeup artist and/or hairstylist might use.

LOCATION

If you booked our Glamour Model Experience, or you chose one of our TFP experiences and purchased hair and makeup service, and you select Outdoors, you will need to come to our studio for hair and makeup before we go to the outdoor location. Outdoor locations must be within a 1.5-hour drive (100-120 miles from Harry Reid International Airport).
Which indoor location do you prefer?
You are responsible for booking the location and all rental costs, including cleaning fees, booking fees, and taxes if you choose to have your session in an indoor location other than Phantasy Photos' studio or your home.

OUTDOOR LOCATIONS

Not all locations listed above are suitable for boudoir and nude sessions. Outdoor boudoir and nude session locations will be as secluded and discreet as possible, but with any outdoor location, there is the possibility of being seen.
There may be entry fees to some locations. You are responsible for paying entrance fees for yourself and all Phantasy Photos personnel.

SELF-EVALUATION

In this section, you are asked to answer questions about yourself. Be honest. If you can't be honest with yourself, we know you won't get everything out of your Dream Boudoir Experience that you deserve. And it won't be near as fun. So, please take your time, think carefully about your answers, and be honest with yourself. We aren't here to judge you. We're here to lift you up and help you feel good about yourself!

Talk about areas of your body that you dislike. Tell us what concerns you about those body parts. Talk about insecurities you might have. Are you anxious, nervous, and concerned about working with a male photographer? Let's talk about it. Tell us about any concerns, large or small, you might have so that we can discuss them in advance of your dream boudoir experience. If you don't have any concerns, just enter None or something like that.
Not required, but answering will help us learn what elements of the setting you like. This can be anything: poses, lighting, setting, clothing, hair, makeup, props, location. etc. If we know what you like and want, we can make sure to include it in your experience.
Feel free to enter any additional comments, suggest ideas you may have for your experience, or ask questions you have concerning your experience.

If you can't be honest with yourself, we know you won't get everything out of your Dream Boudoir Experience that you deserve. And it won't be near as fun. Turning Up the Heat

It's up to you and your partner how far you want to go. We have five (5) levels of heat:

  1. Jalapeño Pepper - clothing covers penises, vaginas, and breasts (female-identifying persons), from fully clothed to undergarments/lingerie only. Male-identifying persons may be topless.
  2. Habanero Pepper - clothing includes sheer clothing, with which naughty bits are covered but somewhat visible, implied topless (for female-identifying persons), and implied nude. Male-identifying persons may be topless.
  3. Ghost Pepper - topless or nude, in which penises and vaginas may be partially exposed. Sex may occur but only photos that suggest sex is occurring are taken.
  4. Trinidad Scorpion Pepper - explicit nudity, in which penises, vaginas, and anuses are partially or fully exposed. Sex may occur, but only softcore sex photos are taken.
  5. Carolina Reaper Pepper - erotica and sex. Hardcore sex photos are taken, including oral sex, vaginal sex, anal sex, etc.

All heat levels may include elements of BDSM. You must supply the BDSM accessories.

Magazine Publication

Your Dream Boudoir Experience gives you the opportunity to have one or more photos published in Phantasy Photos' yet-to-be-named boudoir magazine.

Will you allow Phantasy Photos to publish one or more photos in its yet-to-be-named boudoir magazine?
Supermodel Experience

If you opt-in, you'll grace the cover and be exclusively featured in Phantasy Photos Supermodel Special Edition magazine. We'll conduct an in-depth interview with you to learn more about you, your work, your hobbies, your passions, any causes and charities you support, and more. This interview will be published in the magazine along with a bio we'll ask you to write and many of the photos we created during your photo session, including some behind-the-scenes photos. Finally, you'll be eligible to be named Phantasy Photos Supermodel of the Year.

In addition, you'll receive the following:
  • Digital copy of the magazine cover
  • One 20x30 framed poster print of the magazine cover
  • One digital copy of the magazine
  • One printed copy of the magazine
Cover Model Experience

If you opt-in, you'll grace the cover of an issue of Phantasy Photos yet-to-be-named boudoir magazine. You'll also have an 8-page feature, complete with your comprehensive bio and a few fun facts about you. You'll also be eligible to be named Phantasy Photos Cover Model of the Year.

In addition, you'll receive the following:
  • Digital copy of the magazine cover
  • One 16x20 framed poster print of the magazine cover
  • One digital copy of the magazine in which you are published
  • One printed copy of the magazine in which you are published
Glamour Model Experience

If you opt-in, you'll get a 4-page feature in an issue of Phantasy Photos yet-to-be-named boudoir magazine, complete with your bio and a few fun facts about you. You'll also be eligible to be named Phantasy Photos Glamour Model of the Year.

In addition, you'll receive the following:
  • One digital copy of the magazine in which you appear
  • One print copy of the magazine in which you appear


Complimentary Boudoir Experience

Part of the agreement for receiving our Complimentary Boudoir Experience is allowing us to share and use your photos in our yet-to-be-named boudoir magazine. As such, you'll get a 2- to 4-page feature in our magazine, complete with your bio, a few fun facts about you, and several photos. You'll also be eligible for our Phantasy Photos New Model of the Year. Finally, you will receive one digital copy of the magazine in which you appear.
Instagram Model Experience

If you opt-in, you'll get a 2-page mini-feature in an issue of Phantasy Photos' yet-to-be-named boudoir magazine, complete with a short bio and a few fun facts about you. You'll also be eligible to be named Phantasy Photos' Instagram Model of the Year. Finally, you will receive one digital copy of the magazine in which you appear.

Mini Boudoir Experience

If you opt-in, you'll have one photo published in an issue of Phantasy Photos yet-to-be-named boudoir magazine.
Optional, 1-Hour, On-Location, Professional Massage

Your Dream Boudoir Experience includes a 1-hour, on-location professional massage after lunch. It is completely optional.

We book the masseuse shortly after we receive your booking, so you may not change your mind once you submit this questionnaire.

Your Dream Boudoir Experience includes a relaxing bath while you wait for your photo reveal. It includes sparkling cider or other non-alcoholic beverage of your choice, your choice of a box of Ethel M Chocolates, and your choice of bubbles or bath bomb. It is completely optional.
